A very common exam question especially those going for the part 1 and 2a exam for MRCP
This 20 years old man who presented with this rash and 1st degree heart block.
What is the diagnosis and what is the treatment ?

erm, not so sure, but would like to try...
this rash is erythema marginatum which suggestive of rheumatic fever, in which the patient may have prolonger PR interval.

This rash is erythema migrans(bull's eye rash) associated with Lyme disease.The heart block is probably due to manifestation of the heart.
Treatment: antibiotics-doxycycline, if have later complications-cefotaxime or ceftriaxone.

this is chronicum erythema migrans of lyme disease associated with arthritis ,CNS involvement and heart arrhythmia or block
TTT is cefitrioxone, doxycycline
