what sign is it? splaying of lower molars?

gum hypertrophy? but it's not too obvious

causes: ciclosporin, phenytoin, nifedepine, leukemia (e.g acute myeloid leukemia), sarcoidosis etc...

Yes, it is gum hypertrophy. This patinet has epilepsy and in on C.Phenytoin for a long time.

how do you objective diagnose gum hypertrophy?

I don't have an answer for that. For all I know, it is a clinical observation, such as looking for clubbing.
